COVID vaccinations avaialble by appointment in Glades County

MOORE HAVEN — The Department of Health in Glades County will provide COVID-19 Vaccines by appointment on Sunday, March 7 and Monday, March 8 in Moore Haven. The health department continues to use the state online registration system for pre-registrations. Please do not call the direct phone number for the Health Department. You must use the online registration system listed below.

Individuals can pre-register to “get in line” and will be notified with available appointment time. Visit https://myvaccine.fl.gov/ or call 866-201-6745.

Please note:
• Do not arrive more than 15 minutes prior to your appointment.
• Do not bring pets (only service animals are allowed inside health dept).
• You will be required to fill out local health department paperwork on site.
• Your pre-registration is not your appointment.
• You will be notified by phone of your appointment time.
• All clients receiving a vaccine must be registered.

The Health Department will follow Executive Order 21-47 which expands COVID-19 vaccination in Florida to include the following: During this first phase of vaccine administration, all providers administering any COVID-19 vaccine shall only vaccinate the following populations:

• Long-term care facility residents and staff;
• Persons 65 years of age and older;
• Health care personnel with direct patient contact;
• K-12 school employees 50 years of age and older;
• Sworn law enforcement officers 50 years of age and older; and
• Firefighters 50 years of age and older.

The Health Department will also follow the State Surgeon General’s Public Health Advisory, which requires every vaccine recipient to be a resident of Florida or is a frontline healthcare worker providing services in Florida.

Florida Residency Requirements
You will need a valid Florida Driver License or Florida Identification Card OR two of the following items:

  • A deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental or lease agreement. (Copies of deeds can be found on the County Property Appraiser website)
  • A utility bill or utility connection/work order that is not more than two months old.
  • Mail from a bank or other financial institution, including checking, savings, or investment account statements, that is not more than two months old.
  • Mail from a government agency that is not more than two months old. (Mail from a government agency can include tax bills showing a Florida address. Tax bills can be found on the County Property Appraiser website).
